For Malaysian Students
2 recent passport-size photographs of student.
1 copy of student’s birth certificate.
1 copy of student’s Identity Card.
1 copy of the father’s and mother’s identity card.
1 official copy of the most recent school academic report from previous school attended.
1 official copy of the School Leaving Certificate from previous school attended.
Student’s vaccination table (birth and onwards).
For International Students/ Non-Malaysians
7 recent passport size photographs of student (regulation shape and blue background for Malaysia).
1 copy of student’s birth certificate.
1 copy of the student’s passport (front-cover-to-back-cover, including all pages).
1 copy of the father’s and mother’s passports (cover- to-cover, all pages).
1 official copy of the most recent school academic report from previous school attended.
An official copy of the School Leaving Certificate from previous school attended.
A copy of parent’s marriage certificate. (Notarised documents in English)
